(1) Fill out the registration form at the end of this form. When you click Submit, you will be emailing your registration to the MBAO office. You will get a confirmation email of your registration within 72 hours. If you do not get this confirmation contact our office by email. firstname.lastname@example.org or 559-449-9035.
(2) Print your registration confirmation at the end of this form. This will be your only chance to get a printed confirmation before the conference. You will get your actual credit card receipt at the conference itself, and it will be on the back of your registration card.
(3) All Presenters –
there is a presenter’s form on the first page of the MBAO Web Site that you MUST
attach to your presentation when you email to Gary Obenauf at email@example.com
Take a look at this today please so you know where it is.
This is new and a required form !!!
(5) Refund Policy In order to get the early registration fee of $500.00 you must register by October 25, 2012.
the date of October 25, 2012, the fee will be $600.00.
If you cancel your participation to the Conference prior to October 25th there is no penalty.
No refund for cancellation after October 25th.
(6) Proceedings will be available on CD. Reducing the number of printed hard copies will help reduce cost of the conference. If you are willing to take a CD copy of the proceedings rather than a hard copy please mark the appropriate box on the registration form. It will be a lot easier to take a CD home rather than the printed proceedings.
